Greetings from the Cooperative Development Authority!
This is to inform our valued clients that the Cooperative Development Authority is implementing the Appointment System because of the Inter-Agency Task Force for the Management of Emerging Infectious Diseases (IATF) declaring Alert Level 3 in Eastern Visayas due to the increasing number of COVID-19 affected individuals and to enable the transacting public to conveniently schedule their appointments/visits to the particular Section/Units to process and address their concerns. For walk-in clients, the Office is strictly implementing the established health protocols, to wit:
- Valid vaccination card for those who are fully vaccinated; or
- RT-PCR test with negative result within 24 hours for unvaccinated clients;
RT-PCR presented for this purpose shall be taken one day prior to the visit to the office of the unvaccinated clients.
3. The Valid Vaccination Card and/or Negative result shall be presented to the guard on duty; and
4. Scan QR Code to access entrance to the office premises.
The following minimum safety measures standards shall be observed and followed in the office premises, such as, but not limited to:
- Strict social distancing
- Wearing of face masks
- Hand Hygiene
- Taking of body temperature
- Bring own ballpen
