We would like to inform our valued clients that the Online – Cooperative Annual Progress Report Information System (CAPRIS) is undergoing revision.
Meanwhile, you can download the CAPR form Revision 5 from our website and encode using the excel format and submit it manually to the extension office where you are registered.
However, once the system is operational, you are required to encode through online the previously submitted CAPR as one of the requirements for COC issuances.
