Cooperative Development Authority Regional Office 12 has launched an online request for appointment aimed at providing an alternative avenue for clients all over Region 12 or even other regions across the country to conveniently convey their queries, issues and concerns to the Authority specially during this ongoing corona virus crisis.

Through the help of google forms via bit.ly/3fkNjwE, clients of CDA Regional Office 12 can now request for an appointment online to any of the CDA 12 personnel to attend to their issues, queries and concerns from the comfort of their homes and offices.

It is stipulated in the form that upon submission of their request, they shall be contacted immediately through their selected medium of communication they wished to be connected as specified on their request whether through their contact number or email address. This is to go over details of their request and the availability of the concerned CDA personnel they wish to have appointment with.
